MEDICATION SAFETY

bulletPharmacoepidemiology- I specialize in formal causality assessment (the probability that a drug caused the injury) and use pharmacology, the biomedical literature review, and a validated assessment instrument to assess causality.

bulletMedication Error Prevention – Do you want to know how safe your health care facility or pharmacy is for medication mishaps – to include medication errors, allergic drug reactions, drug interactions, and adverse drug reactions (ADRs)? How close are you to meeting national patient safety goals (JCAHO)? Through survey design and/or a site visit, your facility can be compared to other facilities of similar size and type. Recommendations are formulated on how to better detect problems before they turn into “incidents,” how to better detect, document, and report adverse medication events (ADEs) when they occur, and how to prevent them from occurring in the future.

bulletThe Investigation of Medication Mishaps – Would you like to know if: 1) a medication was the cause of harm? 2) which medication caused the harm? 3) how the medication mishap occurred? or 4) what can be done to prevent it from occurring again? The mishap can be investigated at the site, through a review of medical records, and through discussions with key people who may know what happened. Someone not employed by the institution where the event occurred is in the best position to investigate these situations without bias.

bulletHelp in Meeting National Medication Safety Goals - I can help you assess and meet JCAHO's national medication safety goals.
